Job summary

Praxis Care in Omagh is seeking a Team Leader (39 hours per week) to lead a team of Support Workers in the Omagh Supported Living Service. You will design individual support programmes and coordinate delivery to meet the practical, emotional, and personal needs of the people we support.

This role requires effective leadership, liaison with families and multi-disciplinary teams, and ensuring records, medication management, and regulatory compliance are upheld.

Qualifications

• Essential: 2 years’ experience in a health and social care setting and a commitment to undertake a professional qualification, OR
• Qualification in Social Work, First Level Nursing, Allied Health Profession, Level 5 Diploma in Leadership and Management for Health and Social Care OR
• 1 year paid experience in HSC setting with a commitment to undertake a professional qualification and a Psychology Degree

Responsibilities

• Plan, coordinate, and implement safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Develop and implement individual and group programmes of support to maximize independence.
• Coordinate ongoing assessment and review with suitable models.
• Support the physical, emotional, and personal needs with respect, privacy, dignity, fulfilment, independence, and choice.
• Ensure medication is stored and administered per policy.
• Provide leadership, supervision, and management to Support Workers; monitor time and attendance.
• Participate in learning and development and address personal development opportunities.
• Maintain records and produce reports per policy and statutory requirements.
• Support manager with regulatory compliance.
