Job summary

Praxis Care is seeking a Team Leader to coordinate a team of Support Workers, delivering person-centred care to individuals within the service. You will design individual and group programmes and liaise with families and MDTs to promote needs and independence.

You will supervise staff, manage records, and ensure medication is stored and administered according to policy and statutory procedures, while upholding dignity and respect.

Qualifications

• Minimum 2 years' experience in health and social care setting.
• Commitment to undertake a professional qualification.
• Qualification in Social Work, 1st Level Nursing, Allied Health Profession or Level 5 diploma in leadership and management.
• Right to Work in the UK.

Responsibilities

• Plan, coordinate and implement transitions into the service.
• Lead and supervise Support Workers and manage time/attendance.
• Liaise with families and multi-disciplinary teams to support needs.
• Develop individual and group programmes to enhance independence.
• Maintain records and produce reports per policy and statutory requirements.
• Ensure medication handling complies with policy and regulations.
• Support regulatory compliance and ongoing professional development.
