Job summary

West Sussex County Council is seeking a Care and Support Broker to join the CPST Direct Payments team in Bognor. You will help customers access the right care and support services and work across health and social care to plan and source support during hospital discharge and for assessed needs.

The role is office-based, 37 hours per week, with standard hours 9am-5pm, Monday to Friday. Strong communication, negotiation, and IT skills (especially Excel) are essential, with the ability to manage

Qualifications

• Planning and arranging customer-focused services for people with care and support needs.
• Relaying information to service providers to enable appropriate services to be sourced quickly.
• Organising and prioritising work, working independently and under pressure to tight deadlines.
• Interpreting information to determine risks and communicating safeguarding and service quality issues.
• Excellent written, verbal, influencing and negotiation skills.
• Strong IT and keyboard skills.

Responsibilities

• Plan and source support for people discharged from hospital.
