Salary: £26,790.40 (40 hours per week) + on-call payments (OTE £31,500 per annum)
We are looking for an experienced and compassionate Care Coordinator to join our team on a 12-month fixed-term maternity cover contract, starting in September.
This is an exciting opportunity for someone who is passionate about building strong partnerships across health and social care to ensure people can return home safely and receive the support they need.
The primary focus of this role is to work closely with Great Western Hospital (GWH) and community healthcare professionals, coordinating referrals and supporting smooth, timely transitions from hospital to home.You will play a key role in developing and maintaining professional relationships with discharge teams, community services, and local healthcare partners, ensuring individuals receive responsive, person-centred care when they need it most.
You will work collaboratively with a wide range of professionals including Hospital Discharge Teams, Social Workers, Occupational Therapists, GPs, District Nurses, Community Healthcare Teams, and other partner agencies to coordinate care packages, respond to referrals, and promote positive outcomes for customers living independently at home.
Alongside this, you will support the day-to-day coordination of our domiciliary care service, helping to ensure high standards of care delivery and continuity for both customers and care staff.
