Westgate Healthcare is seeking a compassionate Care Practitioner for a 9-month fixed-term maternity cover role in the Chorleywood area. The position involves 44 hours per week (8:00am–8:00pm), working four shifts with alternate weekends, starting mid-October 2026.

You will provide person-centred care, administer medications, maintain care records, support induction of new staff and contribute to care plan reviews while collaborating with GPs and the wider team.

Qualifications

  • Previous experience in a care home or working with the elderly is essential.
  • QCF/NVQ Level 3 in Health and Social Care.
  • Medication trained.

Responsibilities

  • Providing high-quality, person-centred care and supporting residents with daily living.
  • Safely administering medication in accordance with prescribed instructions and procedures.
  • Completing accurate care records and required documentation daily.
  • Supporting induction, training and supervision of new care team members.
  • Managing, monitoring and reviewing residents’ care plans to ensure accuracy and responsiveness.
  • Maintaining clear daily records and communicating relevant information.
  • Liaising with GPs and healthcare professionals to ensure proper care.
  • Collaborating with colleagues to uphold high care standards and a positive environment.

We are looking for a compassionate and caring Care Practitioner to join our team. This is a 9-month fixed-term contract to cover maternity leave, with the possibility of an extension. The role is due to start in mid-October 2026.

About The Role

44 hours per week, 8:00am – 8:00pm

Working 4 shifts a week, with a requirement to work alternate weekends

  • Providing high-quality, person-centred care and supporting residents with all aspects of daily living, while promoting their dignity, independence and wellbeing.
    • Safely administering medication in accordance with prescribed instructions, policies and procedures.
    • Completing accurate and timely care records and all required documentation on a daily basis.
    • Supporting the induction, training and supervision of new members of the care team, sharing your knowledge and experience.
    • Completing, monitoring and reviewing residents’ care plans to ensure they remain accurate, current and responsive to changing needs.
    • Maintaining clear and comprehensive daily records, ensuring all relevant information is communicated effectively.
    • Monitoring and reporting on residents’ health, wellbeing and changing needs, escalating any concerns appropriately.
    • Liaising effectively with GPs, healthcare professionals, Support Managers and other relevant colleagues to ensure residents receive the appropriate care and support.
    • Working collaboratively with colleagues to maintain consistently high standards of care and contribute to a positive and supportive environment for residents and staff.
