Job summary

Westgate Healthcare in the United Kingdom is seeking a compassionate Care Practitioner to join our team. You will provide high-quality, person-centred care to residents, promote dignity and independence, and work to ensure wellbeing.

The role involves night shifts 8:00pm–8:00am (39 hours per week) with a paid handover, 3–4 shifts weekly, and alternating weekends. You will support medication administration, care planning, and maintain accurate records, while mentoring new team members.

Responsibilities

• Providing high-quality, person-centred care and supporting residents with all aspects of daily living, while promoting their dignity, independence and wellbeing.
• Safely administering medication in accordance with prescribed instructions, policies and procedures.
• Completing accurate and timely care records and all required documentation on a daily basis.
• Supporting the induction, training and supervision of new members of the care team, sharing your knowledge and experience.
• Completing, monitoring and reviewing residents’ care plans to ensure they remain accurate, current and responsive to changing needs.
• Maintaining clear and comprehensive daily records, ensuring all relevant information is communicated effectively.
• Monitoring and reporting on residents’ health, wellbeing and changing needs, escalating any concerns appropriately.
• Liaising effectively with GPs, healthcare professionals, Support Managers and other relevant colleagues to ensure residents receive the appropriate care and support.
• Working collaboratively with colleagues to maintain consistently high standards of care and contribute to a positive and supportive environment for residents and staff.

About The Role

39 hours per week

8:00pm – 8:00am, including a 15-minute paid handover

Working 3–4 shifts per week
