Job summary

Westgate Healthcare is seeking a compassionate Care Practitioner to join our team. You will provide person-centred care for elderly residents, supporting daily living, medication administration, and accurate record-keeping.

A strong focus on dignity, independence and wellbeing is essential. The role requires 39 hours per week, 8:00pm–8:00am with a paid handover, and 3–4 shifts per week including alternate weekends.

  • The right to live and work in the UK.
  • Previous experience in a care home or working with the elderly is essential to be considered.
  • QCF/NVQ Level 3 in Health and Social Care.
  • Medication trained.

We are looking for a compassionate and caring Care Practitioner to join our team. About the role

39 hours per week

8:00pm - 8:00am, including a 15-minute paid handover

Working 3-4 shifts per week

Alternate weekend working required

  • Providing high-quality, person-centred care and supporting residents with all aspects of daily living, while promoting their dignity, independence and wellbeing.
  • Safely administering medication in accordance with prescribed instructions, policies and procedures.
  • Completing accurate and timely care records and all required documentation on a daily basis.
  • Supporting the induction, training and supervision of new members of the care team, sharing your knowledge and experience.
  • Completing, monitoring and reviewing residents' care plans to ensure they remain accurate, current and responsive to changing needs.
  • Maintaining clear and comprehensive daily records, ensuring all relevant information is communicated effectively.
  • Monitoring and reporting on residents' health, wellbeing and changing needs, escalating any concerns appropriately.
  • Liaising effectively with GPs, healthcare professionals, Support Managers and other relevant colleagues to ensure residents receive the appropriate care and support.
  • Working collaboratively with colleagues to maintain consistently high standards of care and contribute to a positive and supportive environment for residents and staff.
