Job summary

Archvale Limited is recruiting a Care Navigator to support the practice clinical team at Castlefields Surgery, Stafford. You will signpost patients to the appropriate healthcare professional or service and may undertake reception duties to provide a courteous, professional service.

The role includes data entry, appointment and prescription processing, and collaboration with the administrative team. A proactive, empathetic approach and clear telephone communication are essential, along with

Qualifications

• Experience in a primary care setting.
• Excellent communication and a clear, polite telephone manner.
• Ability to process information accurately and interpret data as required.
• Sensitive and empathetic in distressing situations.

Responsibilities

• Signpost patients to the appropriate healthcare professional
• Answer incoming phone calls and transfer as needed
• Process appointment requests
• Handle repeat prescription requests
• Initiate contact with patients, team members and external agencies
• Enter patient registrations and update records
• Maintain a clean, tidy reception area and support clinical staff
• Attend mandatory training and appraisal cycles
• Contribute to public health campaigns through advice or care
