Job summary

Avery Healthcare Group is seeking a dedicated Care Manager for Ascot Grange Care Home in Sunninghill. You will provide visible leadership on the unit, oversee standards of care, staffing, documentation and resident experience, ensuring care aligns with policies and regulatory standards.

You will lead care planning, monitor risk, support staff development, and be a key contact for residents, families and professionals, while upholding our values of care, respect, and accountability.

Qualifications

• Hold NVQ 3 or equivalent in care for care homes.
• Demonstrate effective interpersonal skills to lead staff when delivering care.
• Commitment to continuing professional development.
• Positive attitude to developing staff and enhancing residents' well-being.

Responsibilities

• Lead the person-centred care planning process, including implementation, monitoring and monthly evaluation.
• Monitor residents' wellbeing and update care plans accordingly.
• Act as safeguarding lead within residential care and escalate concerns appropriately.
• Be a key contact for residents, relatives and professionals, responding to reasonable requests.
