Carrington Blake Recruitment is seeking a practitioner to support care leavers aged 16-25, helping them transition to independent adulthood. You will coordinate services, provide practical and emotional support, and assist with housing, education, employment and health access.

The role involves liaising with social workers, housing providers and health professionals, traveling nationally when required, with 2 days office and 3 days visits or home working.

Qualifikationen

• Experience working with young people, care leavers or vulnerable individuals.
• Knowledge of safeguarding, child protection and risk assessment.
• Understanding of leaving care services and pathway planning.
• Knowledge of housing, homelessness, education, employment and health support services.
• Experience working with multi-agency professionals and managing complex cases.
• Strong communication, organisation and relationship-building skills.
• Ability to respond to crisis situations and work independently.
• Full UK driving licence and access to a car.
• Willingness to undertake national travel and occasional overnight stays when required.

• Care Leaver Support: Build trusting relationships and provide ongoing practical and emotional support.
• Pathway Planning: Help young people achieve their independence goals.
• Safeguarding: Identify risks and respond to safeguarding concerns appropriately.
• Multi-Agency Working: Coordinate support with social care, housing, education, employment and health services.
• Housing Support: Help young people access and maintain suitable accommodation.
• Youth Engagement: Motivate and support young people who may be reluctant to engage with services.
• Crisis Intervention: Respond to urgent needs and help prevent homelessness or other negative outcomes.
• Case Management: Maintain accurate records, monitor progress and review support plans.
