Job summary

We Do Social Work is seeking an experienced Care Leaver Personal Adviser / Social Worker to join Children’s Services in England. You will support care leavers preparing to leave care or already transitioning, promoting independence and ensuring access to practical, emotional and financial support.

You will manage a caseload, develop person-centred Pathway Plans, work with young people, families and partner agencies, and maintain accurate records, safeguarding and reviews.

Qualifications

  • Experience working with care leavers/young people transitioning to adulthood.
  • Experience managing an allocated caseload.
  • Knowledge of safeguarding procedures and risk assessment.
  • Experience developing and reviewing Pathway Plans.
  • Ability to work in a multi-agency environment.
  • Strong communication and advocacy skills.

Responsibilities

  • Provide practical advice and support on accommodation, budgeting, education and health for care leavers.
  • Complete, monitor and review Pathway Plans to reflect young people’s aspirations.
  • Support independence and decision-making for care leavers.
  • Facilitate and supervise contact with birth families as required.
  • Coordinate services and advocate for care leavers’ needs.
  • Maintain regular contact and monitor progress, welfare and wellbeing.
  • Identify risks and escalate concerns per safeguarding procedures.
  • Record information securely and maintain case files.
