Job summary

Hertfordshire County Council is seeking a Leaving Care Personal Advisor to support Care Leavers aged 18–25 at Farnham House, Stevenage. You will lead assessments, plan pathways and coordinate multi-agency support to help young people transition to adulthood.

This permanent, full-time role requires safeguarding knowledge, strong communication and the ability to manage a complex workload. Driving and relocation flexibility may be needed to support duties.

Qualifications

• Knowledge of child development, adolescence, social, emotional and mental health needs.
• Knowledge/experience of working with care leavers, young people and/or families.
• Knowledge of current legislation and guidance relating to safeguarding children and adults.
• Experience of multi-agency working with various statutory agencies.
• Ability to communicate effectively with young people, families and professionals.

Responsibilities

• Hold a caseload of Care Leavers aged 18–25 and act as lead professional.
• Develop pathway plans and risk assessments with other agencies where appropriate.
• Coordinate multi-agency support including mental health, housing and substance use.
• Provide direct support for transitions to independent living, including transport and moving arrangements.
• Monitor progress, keep records and produce reports to required timescales.
