Job summary

• Experience providing direct care or clinical support in an NHS setting.
• Ability to undertake delegated clinical observations and interventions after training.

Responsibilities

• Ward rounds and MDT support with delegated observations.
• Personalised care and support planning across mobility, nutrition, hydration, continence and wellbeing.
• Monitor residents and recognise deterioration; escalate to registered clinician.
• Support long-term conditions and preventative care; participate in end-of-life care planning.
