Job summary

Kent Community Health NHS Foundation Trust is recruiting Healthcare Assistants to support housebound patients in their own homes across the region. You will deliver care in line with personalised plans, visit daily, document care and escalate concerns to the wider team.

The role offers training and development opportunities; shifts typically between 08:00 and 18:00, seven days a week including bank holidays, with potential flexible arrangements. Right to work in the United Kingdom is required.

Qualifications

• Experience in holistic assessments and care planning is beneficial.
• Strong documentation and handover skills are important.
• Ability to work with multidisciplinary teams to deliver patient-centred care.

Responsibilities

• Visit housebound patients in their own homes and deliver care per personalised plans.
• Document care and escalate concerns to the wider team as needed.
• Take part in daily handovers and support additional patient-related tasks.
• Opportunity to take on champion or link roles and contribute to team development.
• Service operates 7 days a week with shifts typically 08:00–18:00, with some flexibility.
