Job summary

Epsom and St Helier University Hospitals is seeking a Care Home Pathway Coordinator to support discharge coordination from St Helier Hospital to care homes. You will work with the Transfer of Care Hub (TOCH) to manage the Red Bag journey and ensure timely, safe patient transfers.

The role focuses on improving patient flow, reducing DTOC, and avoiding unnecessary readmissions while coordinating with care homes and hospital teams across Sutton and South West London.

Qualifications

• Experience in care home discharge or hospital discharge coordination.
• Ability to liaise with multiple stakeholders including care homes and hospital teams.
• Strong focus on patient flow and timely discharges.

Responsibilities

• Coordinate all aspects of supported discharges and single point of discharge from St Helier Hospital to care homes.
• Work closely with the Transfer of Care Hub (TOCH) and other stakeholders to deliver the Hospital Transfer pathway.
• Ensure Red Bags are received in hospital and returned to care homes on discharge.
• Support reduction of delayed transfers of care (DTOC) and readmissions.
