Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a specialist Housing Support Lead to oversee housing across inpatient and community pathways, ensuring safe and timely discharge.

You will manage a small caseload of the Trust's most complex housing-related discharge cases, providing expert advice to multidisciplinary teams and coordinating with Local Authorities and housing providers to remove barriers such as homelessness and tenancy issues.

Qualifications

• Educated to degree level or equivalent experience in housing or related field.
• Evidence of Continued Professional Development.

Responsibilities

• Provide specialist leadership for housing across inpatient and community pathways, ensuring housing needs are identified and addressed from the point of admission to support safe and timely discharge.
• Manage a small caseload of the Trust's most complex housing-related discharge cases, providing expert advice to multidisciplinary teams.
• Coordinate with Local Authorities, housing providers and partner agencies to resolve barriers such as homelessness and tenancy issues.

Education
