Job summary

Abbots Care is seeking a Care Coordinator to manage the organisation and delivery of care, mainly at weekends and evenings, for service users in their own homes. You will lead a team of care assistants, ensuring person-centred care and engagement with commissioners and local authorities.

Role involves rostering, reporting to senior management, and supporting teams on a rostered on-call basis, with a focus on maintaining capacity and high-quality outcomes for service users.

Qualifications

• Experience coordinating and rota management.
• Vibrant, self-motivated and passionate about making a difference.
• Excellent verbal and written communication; comfortable on the phone and writing emails.
• Experience in a fast-paced environment and flexible.
• Strong attention to detail; a completer/finisher.
• Team player with strong interpersonal skills.
• Holding a full driving license and access to your own vehicle.

Responsibilities

• Rostering and daily management of Care Staff who are based in the community.
• Working with multi-disciplinary teams and professional bodies to ensure outstanding care.
• Complete quality calls with care staff and service users.
• Provide reports to senior management as and when required.
• Provide support to our teams as part of a rostered-on call system.
• Manage referrals from commissioners and local authority.
• Ensure full capacity is utilised.
