Caremark Kirklees in Huddersfield is seeking a dedicated Care Coordinator to become the organisational heart of the office. You will coordinate care, build reliable rotas, and be a trusted contact for customers, families and carers, ensuring the right carer is in the right place at the right time.

It's a fast-paced, people-focused role where your work truly makes a difference. You'll work Monday to Friday, 9am–5:30pm (37.5 hours) with an on-call weekend rota, supporting recruitment, maintaining

Qualifications

• Experience in care coordination or scheduling in a busy setting.
• Proven ability to organise and manage time effectively.
• Excellent communication with customers and colleagues.
• Strong IT skills and confidence with scheduling software.
• Full UK driving licence with access to a vehicle.
• Desirable: background in home care or health and social care.

Responsibilities

• Build and maintain staff rotas to match carers with customers.
• Be the first point of contact for customer and carer queries.
• Monitor care delivery to meet high standards and regulatory requirements.
• Coordinate with customers, families, healthcare professionals and local authorities.
• Support recruitment, onboarding and retention of care staff.
• Keep accurate records and ensure CQC compliance.
• Handle scheduling changes and provide on-call cover when needed.
• Take part in on-call rota 1 weekend in 6.
