Job summary

Agincare Weymouth is seeking an experienced Care Coordinator to schedule and match care workers with clients across the South West. You will ensure continuity of high-quality care and support client starts while coordinating complex schedules.

The role requires attention to detail, organisation, and flexibility to participate in evenings/weekends on-call rotations. Join a growing, values-driven care provider with strong training and development opportunities.

Qualifications

• Must have at least 12 months right to work in the UK.
• Great level of attention to detail and well organised.
• Must be available to work 40 hours per week; office hours 08:30–17:00.
• Part of the on-call rota system during evenings and weekends as required.

Responsibilities

• Plan and coordinate care schedules, ensuring delivery with continuity of care for clients.
• Support new client starts, including matching clients with suitable care workers and coordinating schedules.
• Maintain safeguarding procedures and respond to concerns, allegations of abuse or complaints relating to service delivery.
• Prioritise communication with next of kin, GPs, nurses, physiotherapists and other professionals where this may affect a client's care.
• Keep the Registered Manager informed of relevant concerns, changes or developments.
• Ensure all care worker holidays, sickness, unplanned absences and emergency calls are recorded electronically.
• Work closely with care teams, clients and professionals to resolve scheduling issues and maintain a reliable service.
• Help ensure clients receive the right care, at the right time, from the right person.
