Agincare is seeking a Care Coordinator to connect carers with clients, organise care schedules, and support new packages. You will communicate with families and healthcare professionals to ensure continuity of care across services.

Qualifications

• 12 months remaining on right to work in the UK; valid UK driving licence.

Responsibilities

• Match carers with clients and organise care schedules.
• Support the setup of new care packages.
• Keep rotas running smoothly and ensure continuity of care.
• Communicate with families and healthcare professionals.
• Record staff availability, absences and emergency calls.
• Provide on-call support during evenings and weekends when required.
