Job summary

North Liverpool Primary Care Network is seeking a proactive Care Coordinator to support patients in navigating services and improving outcomes across the network. You will act as an ambassador, promoting PCN work to communities and stakeholders.

You will work with PCN leaders, practice teams and partner organisations to deliver quality improvement, outreach activities and integrated care across North Liverpool.

Qualifications

• ECDL or equivalent Diploma/HNC level or relevant experience.
• NVQ Level 2 in Business Administration or relevant experience.
• Training in motivational coaching and interviewing.

Responsibilities

• Support patients to access appropriate services and improve outcomes.
• Coordinate engagement with PCN and partner organisations.

Main duties of the job

This role offers the opportunity to be at the heart of Primary Care Network development, supporting projects that make a real difference to patients, practices and the wider community. With a varied workload and no two days the same, it is ideal for someone who enjoys building relationships and contributing to innovative healthcare services.

As a Care Coordinator, you will work with PCN leaders, practice teams and partner organisations to improve patient care and access to services. You will support patients with personalised care needs, helping them navigate health and community services and access the right support at the right time.

You will help promote the work of the PCN through social media, patient engagement activities and health campaigns, raising awareness of local services, screening programmes, immunisations and wellbeing initiatives. You will also support the Patient Participation Group, gather patient feedback and contribute to service improvements.

Working closely with the PCN Manager, Clinical Director and partner organisations, you will support quality improvement projects, audits and healthcare programmes while developing partnerships that enhance integrated patient care.

Job responsibilities

• 1.Undertakework in line with PCN directed priorities.

• 3.Proactively identify and work with a cohort of people to supporttheir personalised care requirements, using the available decision support aids
• 4.Support Link worker manager, with appointment audits andcoordinator of outreach work
• 5.Support PCN Neighbourhood leads
• 6.Support PCN Clinical director and PCN manager
• 7.Send Patient surveys for PCN Practices
• 8.Raiseawareness of health promotion in PCN Practices
• 9.SupportPCN Leads with uptake of national screening programmes
• 10.SupportPCN Leads with immunisation programmes
• 11.Supportdelivery of the PCN covid vaccination programme
• 12.Direct liaison with multi agencies to coordinatecare for patients
• 14.Run PCN Extended access audits
• 15.To support patient/carer contact roles, andcollate patient and carer feedback on their experiences
• 16.Support Quality and Outcome Frameworks and otherDES/LES specifications
• 17.Maintain and develop PCN engagement with allpractice staff and encourage best practice
• 18.Perform any other duties assigned by management

Qualifications

• ECDL or equivalent Diploma/ HNC level (or relevant experience)
• NVQ Level 2 Business Administration (or relevant experience) Demonstrable commitment to professional and personal development
• Training in motivational coaching and interviewing or equivalent Oliver McGowan training
• Excel, microsoft office training
• Teams training

Experience

• Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work)
• Experience of data collection and providing monitoring information to assess the impact of services
• Experience of partnership/collaborative working and of building relationships across a variety of organisations Working in a multi-disciplinary setting where influence and negotiation is required Knowledge/familiarity with medical terminology
• Experience of supporting people, their families and carers in a related role (including unpaid work)
• Experience in use of EMIS databases
• Experience of working in the NHS with or in general practice Vulnerable adults awareness Experience of care of the elderly
