We are looking for a Health care assistant to jointhe team on a bank basis, covering all the services within Inclusion Health.
Compass Medical Practice is a GP practice for thosewho are difficult to manage in mainstream practice. Patients are eitherconsulted via telephone/ video link or face to face. When a face-to-faceappointment is required, a risk assessment is undertaken to assess what supportthe clinician requires. Consultations are supported with the use of patientliaison officers and consultations are carried out in our mobile clinic van orin an agreed prebooked venue.
Compass Medical Practice also provides medical coverto temporary patients residing at Thornton House. These are short term patientsthat are residing at Thornton house for either rehab, rest or recouperation. Thereis a weekly ward round and multidisciplinary team meeting. Compass Medicalpractice also offers medical support throughout the week, outside of the wardround, during opening hours.
Our homeless health service is nurse led and isoperates on a Monday, Tuesday, Thursday and Friday from the Salvation Army incentral Blackpool. The nursing team take a multidisciplinary approach, oftenworking with wider services to meet the needs of a complex patient cohort whoare facing multiple disadvantage.
The targeted demand management service offers awound care service in Fleetwood on weekly basis, and a health care assistantled health check service every two weeks.

Day to Day Duties to include, but not exhaustive:
Support theManagement team to ensure all QOF Targets are met
Produce accurate,contemporaneous, and complete records of patient consultation, consistent withlegislation, policies and procedures
Prioritise,organise, and manage own workload in a manner that maintains and promotesquality
Deliver careaccording to local and national guidelines
Obtain and documentconsent for any treatment undertaken and keep the patient at the centre ofhis/her own care, ensuring full participation in decision making
Work within allrelevant policies and procedural guidelines e.g., infection control,chaperoning and risk management
Safeguardingresponsibilities for adults and vulnerable groups, reporting concernsappropriately at the correct time and engaging with services as needed.
Clinical practice:
To support qualifiedclinicians in supporting patients seen within the Complex Lives services
To provide advice for patients with presentingconditions with support of qualified clinicians
To provide Wound Care, undertake ECGS,Observations and Venepuncture to support the trained clinicians.
Patient education and health promotion
Work to attain the service KPIs and targets.
Ensure all patient contact is recorded in atimely manner on the IT systems to enable tracking of patient outcomes, both inreal-time to support ongoing intervention planning and retrospectively tounderstand the impact of the service.
To ensure mandatory training is kept up to date.
Acknowledge andaccommodate barriers to communication and understanding including speech,hearing, language, and emotion.
Flexibility inattending organised educational updates and eagerness to expand on currentknowledge base and cascading new educational information to colleagues wheneverpossible.
Inform the ClinicalManager of any untoward incidents Work within all relevant policies andprocedural guidelines e.g., infection control, safeguarding, chaperoning andrisk management.
