Digital Transformation Care Coordinator

This role is designed to support Digital Transformation within the Southport and Formby Primary Care Network (PCN).
It will involve working centrally at our main Southport office suite, as well as visits to the fourteen practices within Southport and Formby.
The post will support several corefunctions:
- Digital transformation: Support the PCN and its practices inutilising digital solutions such as online consultation systems and digitisedworkflows to improve efficiency and service delivery.
- PCN services: Enable PCN services to digitise workflows andutilise data more effectively to better understand and optimise their services.
- Digital health support: Assist practices in meeting nationaland local contract requirements by providing digital health support for bothpractices and their patients.

This role is advertised as full time but part-time applications will be considered. Hours of work will be agreed between 8am and 6pm Monday through Friday.
Main duties of the job
Key areas of work include but are not limited to:
- EMIS and Digital Systems Support
- Collaboration with PCN management teamsand staff across internal and external teams to drive digital transformationand service improvements
- Patient Support and assisting practices with Health Inequality Reduction
- Drive Service Efficiency by standardising processes and workflows
- Project and Task Management
You will be working in a growing team consisting of both clinical and non-clinical colleagues, so teamwork and good communication skills are vital.

Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.
Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN).
Southport and Formby PCN is a group of 14 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and wellbeing for our local population of around 129,000 people.

This role will play a key part in using digital solutions to develop our data management capabilities, supporting our member practices, and contributing to the delivery of the PCN's long-term Digital and IT strategy.

Job responsibilities
Duties to include the following;
EMIS and Digital Systems Support:
- Create and run searches using EMIS to extract and analyserelevant data.
- Develop and maintain templates, protocols, and concepts inEMIS to streamline practice and PCN workflows.
- Utilise digital clinical tools such as Ardens, iPlato, and Accurx to enhance digital healthcare delivery across practices and the PCN.
- Use Microsoft Office 365 tools for advanced analytics, reporting and visualisation to improve and streamline service managements andreduce dependence on manual reporting and ad hoc fixes.
Digital Transformation via Collaborationand Communication:
- Collaborate with PCN management teams and staff across internal and external teams to drive digital transformation and serviceimprovements including working on identified projects to support with dataquality/reporting and evaluation.
- Building and maintaining relationships with practices by keepingthem up to date on performance measures such as the Sefton Local QualityContract (LQC) and Investment Impact Fund (IIF). This will involve supportingand monitoring performance and reporting this back regularly to keystakeholders.
- Collaborate with existing PCN services, to drive the digitalmaturity across the network.
- Attending relevant meetings for services across theorganisation.
- Identify opportunities for service improvement throughdigital opportunities.
Care Coordination / Patient Supportand Health Inequality Reduction:
- Assist PCN practices in engaging with health inclusion groupsand reducing health inequalities, aligning with the PCN Health Inequalities Strategyand the requirements of the local and national contracts. This may includesupporting practices to risk stratify their patients based on clinical needand/or locally identified priorities.
- Use population health intelligence and management tools toproactively identify patients requiring targeted interventions and support.
- Supporting practices with digital communications via BatchMessages to Patients (SMS Platforms - e.g., Iplato, AccuRx)
- Monitor and report about PCN and/or practice performance onkey measures such as cancer screening rates and other identified prioritieslocally or contractual obligations.
- Drive integrated working across the PCNs services to enhancecollaboration between practices and/or PCN teams
- Promote efficiency and reduce duplication by standardisingprocesses and workflows.
- Maintain and further develop existing systems and processesin place.
- Support the reduction of variation between practices,ensuring consistent service delivery across the network.
Project and Task Management:
- Manage work and communication using Microsoft Office productsto ensure effective coordination and task tracking across projects. Thisincludes Word, Excel and SharePoint, amongst others.
- Monitor and report on digital transformation progress: Trackthe implementation and impact of digital initiatives across the PCN and provideregular reports to management on improvements and areas for furtherdevelopment.
- Have the ability to work on both small and large-scale tasksas and when required, effectively managing your own time and utilising supportavailable as required.

Skills and Knowledge
- Able to build network links with external organisations as an ambassador for the Primary Care Network
- Ability to communicate effectively and professionally with colleagues from a wide variety of healthcare professions and diverse groups
- Creating/Editing/Running Searches and Reports in EMIS
- Data visualisation in excel & power BI
Experience
- Competent with EMIS clinical software
- Experience in complex administrative procedures
- Competent in the use of Microsoft Office applications
- Experience of working in a healthcare setting
- Experience in coordination or project roles
- Knowledge of community health and social care systems
Qualifications
- Educated to GCSE or A Level standard in English and Maths
- Evidence of continued personal development
- Enrolled in, or willing to undertake or previously completed the Personalised Care Institute training
