Norfolk Primary Care C.I.C. is seeking a Non-Prescribing Practice Nurse to deliver high-quality, patient-centred care across practice and community settings, including autonomous home visits.

You will work within the Home Visiting Team alongside GPs and other professionals to support housebound and vulnerable patients. The role requires an NMC registration, autonomous decision-making, and a commitment to reducing health inequalities, particularly for patients with learning disabilities.

This position offers a unique opportunity to work as an autonomous practitioner within a supportive Home Visiting Team, delivering care where it is often needed most. The role combines traditional practice nursing with a substantial community-based element, allowing the successful candidate to build meaningful relationships with patients in their own environments and make a significant impact on the health and wellbeing of housebound, vulnerable, and learning-disabled patients.

Main duties of the job

The Non-Prescribing Practice Nurse will deliver high-quality, patient-centred nursing care across both practice and community settings. This is a highly autonomous role requiring the postholder to manage an independent clinical workload, undertake comprehensive assessments, and deliver evidence-based interventions within their scope of practice.

A significant component of the role involves providing care to patients in their own homes, care homes, and other community settings. The postholder will work closely with the multidisciplinary team whilst exercising professional judgment and clinical autonomy to support housebound, vulnerable, and complex patients.

The role is based within and professionally supported by the Home Visiting Team, working alongside GPs, nurses, paramedics, healthcare assistants, community services, and other healthcare professionals to provide coordinated care closer to home.

The postholder will also play a key role in supporting patients with learning disabilities, helping to reduce health inequalities and improve access to healthcare services.

Job responsibilities
  • Promote health improvement, disease prevention, and wellbeing through evidence-based health education, health promotion, and lifestyle support.
  • Assess, plan, implement, and evaluate person-centred nursing care for patients with acute and long-term health conditions within own scope of practice.
  • Support the management of patients with chronic diseases, undertaking routine reviews, monitoring, and follow-up in accordance with local and national guidelines.
  • Identify patients at risk of developing long-term conditions and provide proactive interventions to improve health outcomes.
  • Deliver routine nursing procedures including wound care, cervical screening (where competent), vaccinations, immunisations, health checks, venepuncture, ECGs, and other clinical interventions within competency.
  • Undertake autonomous home visits, care home visits, and community-based assessments as a core component of the role, managing a delegated caseload and providing assessment, treatment, monitoring, and care planning for housebound and vulnerable patients.
  • Manage an independent clinical caseload across practice and community settings, prioritising workload appropriately and exercising professional judgement when assessing, planning, and delivering patient care within professional competence and organisational policies.
  • Develop and maintain therapeutic relationships with patients, carers, families, and multidisciplinary teams to facilitate coordinated care.
  • Support the identification and reduction of health inequalities, with particular focus on improving access, engagement, and outcomes for patients with learning disabilities.
