Royal Trinity Hospice is seeking an experienced Care Coordinator to join the Clinical Hub Services on a fixed-term contract. The role focuses on coordinating patient care across community settings to achieve the patient’s preferred place of care and death while supporting rapid hospital discharges.

Qualificações

• Competent IT skills and confidence with common software.
• Effective interpersonal and written/verbal communication.
• Strong organisational skills with attention to detail.
• Previous experience in healthcare and/or end-of-life care preferred.
• Willingness to learn new skills and adapt.
• Teamwork with patients and multidisciplinary teams.

Responsabilidades

• Support functions of the Clinical Hub, including First Contact and Rapid Response.
• Coordinate care pathways to ensure timely, appropriate services.
• Act as single point of access to Trinity’s care services for patients and carers.
• Liaise with healthcare professionals to allocate the right service at the right time.
• Support rapid discharges and avoid unnecessary hospital admissions.
• Contribute to high-quality, patient-centred palliative care in the community.
