Job summary

Herts at Home is seeking a proactive Care Coordinator to join our community operations team. The role is fixed term for 12 months, with potential for a permanent contract, and is based at our North Herts office in Letchworth.

You will plan and coordinate care visits, manage rotas, support service users and families, and act as the link between the care team and external professionals. The 40‑hour week runs Monday to Friday 8:00am–5:00pm, with on‑call cover two evenings or weekend shifts per

Responsibilities

  • Planning, coordinating, and scheduling care services to ensure continuity and high standards of care for our service users.
  • Using our electronic scheduling system to create and maintain efficient staff rotas, ensuring care workers receive their schedules in a timely manner.
  • Monitoring care delivery and responding proactively to changes in service user needs, staffing requirements, and operational demands.
  • Producing accurate reports and analysing scheduling data to support service performance and compliance requirements.
  • Providing hands-on care support when required, ensuring excellent standards of care are maintained at all times.
  • Participating in the on-call rota, and responding to operational issues outside of normal working hours.
  • Liaising with local authority brokerage teams and other external agencies regarding new care packages and scheduling changes.
  • Acting as a primary point of contact for service users, families, care staff, healthcare professionals, and social care teams regarding care packages and scheduling queries.
  • Monitoring system alerts, identifying potential issues, and escalating concerns appropriately to the Registered Manager.
  • Supporting the smooth day-to-day operation of the service while maintaining exceptional levels of customer service and care delivery.

Location: North Herts (office based in Letchworth)
Role Type: The role is 40 hours per week, working Monday to Friday from 8:00am to 5:00pm, with a requirement to provide on-call cover for two evening or weekend shifts per month.

Salary: £30,591.48

About the Role

As a Care Coordinator, you will play a vital role in ensuring that our service users receive safe, effective, and person-centred care. You will be responsible for coordinating care visits, managing rotas, and acting as a key link between our care team, service users, families, and healthcare professionals.

  • Liaising with local authority brokerage teams and other external agencies regarding new care packages, changes in care requirements, and service provision.

About You

  • Strong IT skills, including experience with Microsoft Office applications. Knowledge of Nourish or similar care scheduling software would be advantageous.
  • Exceptional communication and interpersonal skills, with the ability to build positive relationships with colleagues, service users, families, and professionals.
  • A calm, professional, and solution‑focused approach, particularly when managing challenging situations.
  • A genuine passion for delivering high‑quality care and improving outcomes for older people within the community.
