GoodOaks Home Care Mid Cheshire is seeking a capable Care Coordinator to join our award-winning team. Based in Mid-Cheshire, you will organise weekly care schedules, balance client requests with carer availability, and maintain clear communication with families and professionals.

You will support staff with scheduling, training, and on-call duties while ensuring high-quality, compliant care delivery and a smooth continuity of service for our clients.

Qualifications

• Full UK driving licence and access to a vehicle.
• DBS check (or willingness to undertake one) and right to work in the UK.
• Experience in care coordination, scheduling or care service delivery is desirable.

Responsibilities

• Create and maintain weekly care schedules, ensuring client visits are covered.
• Manage day-to-day scheduling changes and respond to unforeseen circumstances.
• Handle staff absence, allocate calls, and cancel care calls as needed.
• Assist in training and inducting new team members.
• Allocate care calls and arrange cover where required.
• Conduct care visits and provide field-based support when needed.
• Maintain regular communication with clients, families and healthcare professionals.
• Ensure accurate record-keeping and adherence to policies.
• Help deliver reliable, high-quality care services.
• Participate in the out-of-hours on-call rota when required.
