Qualifications

• Proven Care Coordinator experience in domiciliary care.
• Ability to manage rotas and respond quickly to service changes.
• Level 3 in Health and Social Care (or equivalent).
• Excellent communication, organisation, and time management skills.
• Confident working independently and making informed decisions.

Responsibilities

• Plan, organise, and maintain weekly rotas for care staff.
• Coordinate care visits to ensure timely, safe, and consistent support.
• Build strong relationships with carers, clients, and external professionals.
• Respond to day-to-day changes, ensuring continuity of care.
• Support care workers with queries, updates, and service delivery needs.
• Maintain accurate records and uphold compliance standards.
