Job summary

Solaria Group is seeking a Care Coordinator for domiciliary care in Widnes. This permanent, full-time role runs Monday to Friday, 9am–5pm, with additional on-call duties.

You will ensure high-quality, person-centred care and coordinate staff rotas and care plans. The ideal candidate will have experience in domiciliary care, a good understanding of care plans and CQC requirements, strong organisation and communication skills, and a full UK driving licence with access to a vehicle.

Qualifications

• Experience within domiciliary care, care coordination or a similar healthcare environment.
• A good understanding of care plans, staff rotas and CQC requirements.
• Strong organisational, communication and problem-solving skills.
• The ability to manage competing priorities in a busy care environment.
• A full UK driving licence and access to your own vehicle (essential).
• NVQ Level 3 in Health and Social Care (desirable but not essential).

Responsibilities

• Coordinating daily care services and managing staff rotas to ensure appropriate coverage.
• Supporting and communicating with care professionals, service users and their families.
• Reviewing and updating care plans to ensure individual needs are met.
• Responding to changes in care requirements and resolving scheduling challenges.
• Supporting compliance with CQC regulations and internal quality standards.
• Participating in an on-call rota to support service delivery outside normal working hours.
