A leading mental health organization seeks a care coordinator to join the Early Intervention for Psychosis team. You will support individuals with their initial psychosis experience, work in a dynamic, multi-disciplinary environment, and contribute to their care and recovery. Ideal candidates will have a strong background in mental health care and excellent interpersonal skills.
We are looking for a care coordinator to join our friendly and welcoming Early Intervention for Psychosis team in Salford. You will be joining a large multi-disciplinary team with a wide range of knowledge and experience who will support you in your new role.
As a care coordinator, you will work under the CPA framework as this transforms into the Community Mental Health Framework. You will manage a caseload of people who have been assessed as experiencing their first episode of psychosis and support them as they navigate this journey.
You will contribute to the assessment and duty rotas, and you will receive regular supervision and peer support.
