Job Description:

Purpose of Job:

Support optimal patient care by maintaining and documenting quality care measures, reporting findings to monitor statistical outliers, and managing information efficiently to improve patient and population health. Respond to oversight agencies' demands while maximizing incentives and minimizing penalties by meeting their goals.

Essential Functions:

  1. Implement and maintain quality standards necessary for primary care recognition.
  2. Identify, educate, support, and coach team members on documentation processes related to recognitions.
  3. Coordinate transitions of care through communication with patients, caregivers, clinicians, and facilities according to policies and treatment plans.
  4. Schedule appointments and provide support and education on self-management of chronic illnesses based on evidence-based care and population health.
  5. Facilitate team meetings to discuss high-risk patients and develop initiatives to enhance patient care quality.
Job Qualifications:

Education: High school diploma, medical assistant diploma from an accredited vocational institution, or community college course in medical assisting. Current medical assistant certificate preferred.

Experience: Minimum 3 years of recent experience in a medical facility as a medical assistant preferred. Primary Care Office experience preferred.

Other Skills: Microsoft Office experience required. Current health records with appropriate immunizations (hepatitis B and tuberculosis).

Knowledge:

  • Knowledge of healthcare field and medical office protocols.
  • Knowledge of assisting tasks related to specific medical practices.
  • Knowledge of information to be conveyed to patients and families.
