We are currently seeking a Care Coordinator to join our client’s team in Swindon. This is a permanent contract opportunity, working within a friendly team.
Job Duties:
- Report to the Service Manager and be responsible for the day-to-day running of the service provision to clients.
- Produce detailed work schedules for the Care Workers and provide both written and verbal instructions/guidance to them
- Complete staff rotas; send rotas to staff ensuring continuity of care. Ensure there are no gaps and staff have enough travel time between calls
- Assist with recruitment, induction, training, and supervision of Staff.
- You will be given key clients and it will be your responsibility to keep their information and documentation up to date.
- Dealing with any compliments and complaints, ensure these are in writing and followed up
- Enter payroll hours checking against the rota, send completed spreadsheet on the last day of the month to the accountant.
- Carry out risk assessments under the Health and Safety requirements as directed by the Service Manager.
- Keep the Service Manager informed of the outcomes and issues identified concerning Health and Safety issues.
- You will participate in the on-call rota-this can be flexible based on the needs of the company, your needs and the needs of colleagues.
Requirements:
- Previous experience in a care coordination role or similar position is preferred.
- Knowledge of the Care Standards Act 2008 and other relevant legislation governing the provision of care services.
- Excellent communication skills, both written and verbal.
- Strong attention to detail and organizational skills
- Ability to work independently and as part of a team.
- Proficient in using computer software and systems.
- Full UK driving license.
- Enhanced DBS Check Required (Can be done upon successful placement)
