HCRG Care Group is seeking a part-time Care Coordinator in Preston to join their Multi-disciplinary Team. This role involves supporting patient care by managing referrals and scheduling clinic activities. As a Care Coordinator you will play a vital role as first point of contact for patients and care professionals into our Healthy Young People and Family Services in Lancashire! Dealing with varied and exciting challenges, your excellent customer service skills and experience will be instrumental in ensuring they get the help and support they need.
This is a Part-time role working 30 hours per week, Tuesday - Friday 9-5
This is a hybrid role working remotely and then from our Burnley/Preston Hub
