Job description

Ortus Health is seeking a dedicated Remote Care Coordinator to ensure patients receive comprehensive and coordinated care throughout their healthcare journey. This fully remote, full-time role involves working closely with patients and healthcare providers to facilitate communication, manage appointments, and provide support to enhance the overall patient experience. The ideal candidate will possess strong organisational skills, a compassionate approach to patient care, and experience within a GP practice setting.

This full-time role is ideal for individuals who are passionate about making a difference in people’s lives through effective coordination of care. Career progression and professional development will be actively supported.

Requirements

Responsibilities

  • Serve as the primary remote point of contact for patients, addressing inquiries and providing information regarding services and care options.
  • Coordinate and schedule virtual patient appointments, ensuring timely follow-ups.
  • Maintain accurate electronic patient records and documentation in compliance with healthcare regulations.
  • Collaborate with healthcare professionals to develop and implement individualised care plans.
  • Monitor patient progress and communicate updates to relevant stakeholders.
  • Assist patients in navigating the healthcare system, including understanding processes related to referrals and available community resources.
  • Provide administrative support in a virtual environment, including managing correspondence and maintaining digital records.

Skills & Experience

  • Strong organisational skills with the ability to manage multiple tasks efficiently in a remote setting.
  • Excellent communication skills, both verbal and written, to effectively interact with patients and healthcare professionals.
  • Highly skilled with computers, including proficiency in office software applications and healthcare systems for documentation and scheduling purposes.
  • Empathetic approach towards patient care with a focus on enhancing the patient experience.
  • Ability to work collaboratively within a remote team while also being self-motivated and proactive.
  • Prior experience in a GP practice is essential.

£23,900 per annum based full time hours increasing to £25,000 once training is passed
