A day in the life of a Care Coordinator

The main purpose of the Care Coordinator is to match our carers with clients across our South West region. You will be responsible for tasks related to onboarding new clients, including matching and scheduling carers.

  • You will support forward planning of scheduling to ensure continuity of care;
  • You will understand safeguarding procedures and respond appropriately to any allegations of abuse or complaints regarding service delivery;
  • You will communicate with external sources such as next of kin, GPs, nurses, physiotherapists, and other professionals involved, keeping the Registered Manager updated;
  • You will document all care worker holidays, sickness, unplanned absences, and emergency calls electronically;
  • You will participate in the on-call rota system during evenings and weekends as required.
