Trinity Homecare – Somerset & Wiltshire is seeking a proactive Care Coordinator in Frome to help deliver high-quality community care across Somerset & Wiltshire. You will lead rotas, match carers to clients and respond quickly to changes, ensuring reliable, personalised support.

This role suits an organised, people-focused professional with strong IT skills and a calm approach under pressure. You will collaborate with Care Managers and branch colleagues and contribute to growth of the service.

Qualifications

  • Experience within domiciliary care, homecare, healthcare scheduling, care coordination or another fast-paced people-focused environment.
  • Must have excellent organisation and planning skills.
  • Strong communication and relationship-building abilities.
  • Confidence managing competing priorities.
  • Practical, solutions-focused approach.
  • Ability to remain calm when plans change.
  • Good IT skills and confidence learning scheduling and care systems.
  • Strong attention to detail.
  • Genuine commitment to providing an excellent experience for clients and colleagues.
  • Level 2 or Level 3 qualification in Health & Social Care desirable.

  • Developing positive relationships with clients, families and Care Professionals.
  • Helping identify potential gaps in capacity before they affect service delivery.
  • Participating in the branch on-call rota.
  • Supporting the continued growth and success of the Somerset & Wiltshire service.

What will you be doing?

As Care Coordinator, you’ll play a key role in the day-to-day delivery of our service across Somerset & Wiltshire.

You’ll bring together people, schedules and information to help ensure our clients receive high-quality, dependable care.

Your responsibilities will include:

What are we looking for?

You don’t need to tick every box, but we’d particularly like to hear from you if you have experience within domiciliary care, homecare, healthcare scheduling, care coordination or another fast‑paced people‑focused environment.

You’ll ideally bring:

  • Excellent organisation and planning skills.
  • Strong communication and relationship‑building abilities.
  • Confidence managing competing priorities.
  • A practical, solutions‑focused approach.
  • The ability to remain calm when plans change.
