Job summary

Montana Home Care is seeking a dedicated Care Coordinator in Falkirk and surrounding areas. This full-time role involves leading staff to deliver high-quality, person-centered home care, coordinating rotas, risk assessments, audits, and safety compliance.

You will supervise and develop Support Workers, liaising with social workers and families to achieve optimal outcomes. The role requires SVQ Level 3 in Health & Social Care, at least 2 years in a similar setting, and a valid UK driving license.

Qualifications

  • SVQ Level 3 in Health & Social Care is essential.
  • Minimum of 2 years’ experience in a similar role within a social care setting is essential.
  • Proven supervisory and leadership experience with the ability to inspire and develop a team.
  • Excellent communication and interpersonal skills, with the ability to build trust and effective relationships.
  • Driver with access to a vehicle is essential.

Contract: Full-Time- 39 Hours Per Week

Salary:£27,696 per annum

Full UK driving license is essential

We are seeking an experiencedCare Coordinatorto join our dedicated team at Montana Home Care. This is an excellent opportunity to take on a leadership role withinacare-at-home service, overseeing support inFalkirk and the surrounding areas.

About the Role

As Care Coordinator, you will:

  • Lead and support staff to deliver high-quality, person-centered care to people in their own home.
  • Manage day-to-day operations including staff rotas, risk assessments, audits, and compliance with health and safety.
  • Supervise, mentor and develop Support Workers, undertaking appraisals and performance management.
  • Ensure our Person-Centered Support and Enablement model of care is embedded throughout the service.
  • Liaise with social workers, families and external agencies to achieve the best outcomes for the people we support.
  • Deputise for the Service Manager when required.

About You
