Job summary

CCH Group is seeking a Care Coordinator to help organise and deliver high-quality community care across our network. You will manage care workers, assess and review individual care plans, and supervise staff in the field, with travel involved and a valid driver’s license required.

Working Monday to Friday, 9am–5pm with on‑call duties on a rota. We offer a supportive environment in the largest UK home care provider, with opportunities for growth and recognition within local branches.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
