Allscreens Nationwide Ltd is seeking a dedicated Care Coordinator to join our team. You will organise and manage care plans, ensuring clear communication between clients, families and healthcare professionals while supporting efficient administration.
Key duties include coordinating care plans, acting as primary contact, scheduling appointments, maintaining records, and collaborating with multidisciplinary teams to adapt services and ensure high-quality, compliant care.

Prior office experience is desirable to facilitate efficient administrative and organisational tasks.
Assess people’s needs and facilitate joint working across multidisciplinary teams.
Ensure effective flow of information between health, care, and social support services.
Monitor individuals’ needs and respond to changes promptly.
Manage a caseload of individuals, determined locally according to complexity and support requirements.
Support individuals in making informed decisions about their care and accessing appropriate services.
Follow safeguarding procedures and raise concerns appropriately.
Contribute to reducing the likelihood of acute or crisis care by proactive care management.
Collaborate with colleagues to improve service delivery and support neighbourhood, place, and system priorities.
Strong interpersonal and communication skills, particularly in personalised conversations.
Ability to assess, prioritise, and manage complex care needs.
Knowledge of local health and care systems and services.
Competency in facilitating collaboration across teams.
Flexibility to adapt skills to specialist roles or a diverse range of conditions.
Commitment to safeguarding and ethical care practices.
Care Co-ordinators require support from their employer to succeed in their role. This includes access to supervision, guidance on safeguarding procedures, and clarity on the purpose of the role within local service priorities.
Minimum of three years’ experience in a health or social care role is required.
