Job summary
Do you have experience of working in healthcare, have great organisation and patient facing skills? Are you able to demonstrate good IT skills including Excel, Word, and Outlook?
You will be part of the Care Coordinator Team, you will need to have a flexible approach and adapt with the role as it evolves. You will have excellent communication and administrative skills to enable the collaboration needed to deliver services and support development at network level.

Main duties of the jobCanterbury Integrated Healthcare supports the 7 Canterbury practices and their PCNs covering a patient population of around 100,000 and we have worked together for many years.
The local care vision in East Kent is for localities to develop a model of care that enables people to live as independently as possible by delivering high quality, person centred care that integrates hospital, social care, community and voluntary services around primary care networks of GP practices.
The vision for Canterbury is for the two PCNs to work together to achieve excellent patient care through integrated pathways accessed close to home via local GP practices.

The Care Coordinator Team is primarily responsible for supporting the Canterbury PCNs in the collaborative delivery of services and supporting the care for cohorts of patients. The main focus of the role will be on coordinating the care of identified cohorts of patients and ensuring PCN services are delivered effectively. The Care Coordinator might be required to liaise with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional. You might also be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.

The role will be based mainly at our premises on Ethelbert Road. It will be necessary to have the ability to travel around Canterbury and the surrounding area, between practices and to be able to work remotely some of the time.
