Job Title: Care Coordinator

We are seeking a dedicated Care Coordinator to join our team at CCH Group. In this role, you will be responsible for:

1. Coordinate Care: Managing referrals, creating care worker rotas, liaising with professionals (e.g., social workers, GPs), and monitoring service users' conditions.
2. Staff Management: Supervising care workers, conducting appraisals, handling training referrals, managing HR processes, and approving leave to meet operational needs.
