Job summary

Altogether Care is seeking a Care Coordinator to join our family-focused team. This role involves building strong relationships with clients and their families, coordinating care plans, and delivering high-quality, safe care with consistent hours.

A valid driver's license and access to a personal vehicle are preferred to cover care locations. We offer a permanent contract, full induction, ongoing training, and career development opportunities within a supportive, well-established care provider

Qualifications

• 6 months of care experience or equivalent.
• Able to drive and have access to a personal vehicle.
• Excellent communication and relationship-building with clients and families.

Responsibilities

• Build and maintain positive relationships with clients, families, colleagues and other professionals.
• Utilise technology to deliver quality care.
• Ensure the implementation of safe working practices.
