Job summary

DDEE Care Services is seeking a Care Co-ordinator to ensure the highest quality of support for Service Users. Reporting to the Registered Manager, you will organise caregiver schedules, coordinate needs, and maintain strong communication with Service Users and families.

You will work independently, participate in the on-call system, and contribute to a positive and collaborative team environment while upholding policy and safeguarding requirements.

Responsibilities

• Develop and maintain care staff schedules for Service User needs.
• Match care staff to Service Users based on skills, availability and preferences.
• Address scheduling conflicts promptly to minimise disruption to care.
• Maintain open communication with Service Users, staff, families and health professionals.
• Provide regular updates on Service User condition and care plans.
• Document relevant information accurately in the care management system.
• Address concerns professionally and promptly, with swift resolution.
• Identify potential issues and implement preventative measures for smooth delivery.
• Escalate complex situations to management with recommended solutions.
• Assist with onboarding and training new care staff and ensure understanding of care plans.
• Provide ongoing support and guidance to care staff and promote teamwork.
• Conduct regular performance evaluations and provide constructive feedback.
