Job summary

Erewash Health Partnership leads the Local Navigation hub to coordinate care for people with complex needs in the community. You will work across health and social care to assess needs, navigate referrals and support timely, safe transfers between care settings.

Qualifications

• GCSE English and Maths are required.
• Experience of SystmOne is preferred.
• Experience in a multi-disciplinary health/social care setting is desirable.

Responsibilities

• Coordinate the Local Navigation hub caseload across health and care services.
• Take minutes of MDT meetings and disseminate actions.
• Signpost or refer onward to relevant services.
• Contribute to assessments to identify needs and support independence at home.
• Ensure IT recording requirements are followed in collaboration with the team.
• Provide data support to the EHP data team.
• Promote shared decision-making and use decision tools with patients.
