Job summary

HCRG Care Group is seeking a compassionate Care Coordinator (Administrator) to join the Lancashire Health Visiting and School Nursing Service. You will be the first point of contact for patients, families and professionals, providing a friendly, professional response and directing queries to the right teams.

Based between Preston and Burnley with UK travel, the role offers a salary of £24,853 plus access to a group pension, wellbeing resources and development opportunities to support your career

Qualifications

• Excellent communication and customer service skills.
• Calm under pressure with a high volume of calls/queries.
• Highly organised with strong attention to detail.
• Confident using IT systems and willing to learn new processes.

Responsibilities

• Answering phone calls and emails to the Care Coordination Hub.
• Filter, prioritise and direct queries to the correct teams.
• Book and schedule routine clinic appointments in line with service rules.
• Signpost referrals and enquiries to appropriate services within required timescales.
• Collaborate with colleagues to ensure smooth care coordination.
