• Previous experience in scheduling or care coordination is essential.
• Level 3 in Health & Social Care would be desirable.

Responsibilities

• Rota planning and scheduling to ensure consistent delivery of support visits.
• Provide field-based support when required to meet operational demands.
• Complete comprehensive assessments and care plans with high standards of person-centred care.
• Provide supervision and appraisals to care teams for best practice and development.
• Liaise with clients, families and staff to ensure safe, high-quality care delivery.
• Collaborate with colleagues and other professionals to meet service standards.
