Job summary

Voyage Care is seeking a Care Coordinator for our DCA Office in Caversham. You will coordinate the care delivery team, ensure documentation is in place for new people, discharges and transfers, and monitor live calls to support safe care delivery.

You will liaise with families and healthcare professionals while handling inquiries with confidentiality. This role requires experience in Health & Social Care, strong MS Office skills, and prior administration and customer service experience.

  • Experience in an administration role.
  • Experience providing excellent customer service.

Responsibilities

  • Coordinate care delivery teams to keep people we support safe.
  • Ensure documentation is in place for new people, discharges and transfers.
  • Conduct live call monitoring using bespoke software.
  • Serve as first point of contact for day-to-day operational requirements.
  • Coordinate with families, healthcare professionals and external agencies.

Care Coordinator – Caversham - £15.15 to £16.65 with overtime and evening/weekend enhancements (Ts & Cs apply)

We’re on the hunt for a Care Coordinator to join us in our DCA Office in Caversham.

As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You’ll be responsible for the managing and coordinating of the care delivery team, you will play a key role in ensuring that the relevant documentation is in place for any new people we support, discharges and transfers. You’ll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals and external agencies, handle enquiries, schedule appointments and meetings and handle all information with confidentiality.

This is why at this service our Care Coordinators should have experience working in Health & Social Care, have experience in the use of Microsoft Office and be computer literate, have prior experience in an administration role, and have experience providing excellent customer service.
