Voyage Care is seeking a Care Coordinator in Gloucester to oversee the deployment of care delivery teams and ensure all administration aligns with regulations. This role focuses on coordinating rotas, safeguarding, and maintaining accurate records for the people we support.

Responsibilities

• Coordinate and manage care delivery teams to ensure safe, compliant deployment.
• Ensure all required documentation for new, discharged, and transferred clients is in place and up to date.
• Conduct live monitoring and support day-to-day operational needs of care colleagues.
• Coordinate with families, health professionals and external agencies while maintaining confidentiality.
• Handle enquiries, schedule appointments and meetings.

Location: Gloucester

Pay Rate : £14.34ph

We’re on the hunt for a Care Coordinator to join us in our DCA Office in Quedgeley Gloucester. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!

As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You’ll be responsible for the managing and coordinating of the care delivery team, you will play a key role in ensuring that the relevant documentation is in place for any new people we support, discharges and transfers. You’ll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals and external agencies, handle enquiries, schedule appointments and meetings and handle all information with confidentiality.

You will also take part in the on-call rota (although this may be subject to change depending on the needs of the service). 1 in every 6 weeks
