Job Summary

Part-time role. 18 hours per week. Work pattern to include Thursdays.

Band 4

This role would suit a trained HCA as it involves utilising clinical skills such as phlebotomy, blood pressure checks, and urine dipping in patients' own homes.

We are looking to recruit an additional Care Co-ordinator as part of the multi-disciplinary team in Banbury Alliance Primary Care Network, focusing on reducing health and social care inequalities. This post will work exclusively for Hightown Surgery.

The role offers opportunities for developing leadership and basic clinical skills, as well as face-to-face and remote communication with patients and their families/carers.

This position works closely with our Frailty GP and the local Integrated Neighbourhood Team (INT), which provides holistic, person-centred care within a specific geographic area, especially targeting deprived communities in Banbury.

Main Duties
  1. Orchestrate and navigate care across the health and care system for individuals, especially those with complex needs.
  2. Help connect people with appropriate resources and support at the right time.
  3. Monitor and evaluate care plans, ensuring seamless service transitions.
  4. Manage a caseload of patients, ensuring support is provided and needs are assessed.
  5. Adopt principles of the Comprehensive Model for Personalised Care to improve health and wellbeing.
  6. Support recently discharged patients by developing personalised care plans and facilitating their active participation in care.

About Us

As Care Co-ordinator, you will be the first contact for our frail patients post-discharge, ensuring their care plans meet their needs and arranging necessary follow-ups.

You will handle referrals, build relationships with vulnerable patients, and signpost to relevant services.

Hightown Surgery is part of Banbury Alliance PCN, serving over 12,300 patients, with a multidisciplinary team including pharmacists, physiotherapists, and social prescribers.

We are a training practice with strong links to Oxford University Medical Students. Banbury is a vibrant market town near the Cotswolds, Oxford, Stratford-upon-Avon, Warwick, and Bicester Village.

Job Responsibilities
  • Attend frailty multidisciplinary MDT meetings
  • Assist with referral coordination for community teams
  • Collect data for reports and national databases
  • Liaise with GPs, Practice Management, and staff
  • Conduct telephone and face-to-face meetings with patients, including home visits
  • Occasionally take bloods, collect urine samples, and record basic observations from housebound patients

  • Phlebotomy or Healthcare qualification
