Job overview

The role involves care co-ordination of Service Users, including assessments and delivering interventions to individuals, families, and groups.

You will collaborate with clinicians such as family workers, CBT therapists, psychiatrists, and psychologists.

Driving is essential for this role, as supporting clients to appointments is part of the work. Flexibility in working hours is required to reach clients and carers, with some work outside of office hours. You will work within a small MDT team with a team approach. There may be future opportunities for training in psychological interventions and shared learning with other recovery teams in AWP and the South West.

Main duties of the job

To conduct comprehensive mental health assessments for service users with severe and enduring mental health needs in community settings. This includes:

  • Using standardized assessment tools such as the Cluster Allocation Support Tool (CAST), KGV, and Recovery Star.
  • Assessing history, strengths, aspirations, mental state, cultural and diversity impacts, functional needs, family and carer needs, risk evaluation, physical health, and complicating factors.
  • Determining interventions and treatments to facilitate positive change.
  • Addressing social care needs, safeguarding, public protection, and capacity under the Mental Capacity Act.
